Licensed Vocational Nurse (LVN) – PM Shift (4-2 Rotation)

Yucaipa Hills Post Acute (formerly Calimesa Post-Acute) is an 82-bed skilled nursing facility seeking compassionate and dedicated LVNs for our PM shift. Our mission is to provide exceptional 24-hour care while supporting and valuing our staff.

Position Details

• Shift: PM (3:00 PM – 11:30 PM)
• Schedule: 4-2 Rotation
• Job Type: Full-time
• Pay: Based on experience

Key Responsibilities

• Administer medications and treatments per physician orders and facility protocols.
• Observe, document, and report patient responses to medications and treatments
• Perform patient assessments and contribute to care planning
• Maintain a safe, clean, and comfortable environment for patients and staff
• Follow infection control protocols and promote patient hygiene
• Implement and evaluate individualized care plans
• Assist with discharge planning, including treatments and physician orders
• Maintain a clean, safe, and comfortable environment for patients and staff
• Monitor and report changes in patient condition

Requirements

• Previous experience in a patient care setting preferred
• Strong understanding of the nursing process (assessment, planning, implementation, evaluation)
• Active LVN/LPN license in the State of California
• Current BLS (Basic Life Support) certification
• Commitment to precision, professionalism, teamwork, and high standards of clinical practice
